The final coroner’s report on the death of 'Glee' star Cory Monteith has been released and sadly, there weren’t any surprises.

The coroner’s report says Cory was shooting heroin and drinking champagne the night before he died.  The report says that when emergency responders arrived they discovered a “spoon with drug residue and a used hypodermic needle,” at the scene, as well as two empty bottles of champagne.

The 31-year-old was found dead in his Vancouver hotel room on July 13th. Cause of death was listed as "mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol."

So tragic and so avoidable.
